Face lift (Botulinum toxin) is a toxin derived from the bacterium Clostridium botulinum. Face lift shows its effect by preventing the release of substances that provide conduction at the nerve endings and by stopping the transmission between the nerves and the organs that the nerves reach. If the nerve conduction stops, the function of the organ that reaches the nerve is reduced or completely lost. The mechanism of action of face lift is utilized in many areas of medicine. In the field of plastic surgery, it is generally used to reduce facial streaks caused by the movements of mimic muscles and to reduce sweating in areas of excessive sweating.
Over the years, the mimic muscles work to make the folds on the skin covering it more visible, resulting in dynamic lines on the face. The most common dynamic lines appear on the forehead, between the eyebrows, around the eyes and around the mouth. The lines on the forehead and eye edges give the person an older look and the lines between the eyebrows give the person a frowned, angry look. By applying facial lift to the mimic muscles, the movements of these muscles can be weakened. This results in a significant improvement in the facial expression that appears to be old and angry.
When the face lift is applied to the sweat glands, the conduction of the sweat glands can be reduced by stopping the transmission between the sweat glands and the nerve endings. The sweatiest areas of the body are palms and armpits. Excessive sweating and the smell of sweat associated with sweaty areas of people who apply face lift to improve their complaints.
Face lift is applied as an injection and is not a painful procedure. When a decrease in the movements of the mimic muscles is desired, injection is injected into the mimic muscles and when the sweating complaint is reduced, an injection is injected into the skin. The effect of facelift occurs within the first week after injection and the duration of action is 3 to 9 months, with an average of 6 months. The application can be repeated when the face lift loses its effect. When people who regularly apply face lift for 2 years continue to practice in the same order, there may be a significant weakening of their muscles and a change in facial expression. Therefore, it is useful to perform the applications at longer intervals after 2 years.
Face lift has no serious side effects on health. After the application, a temporary swelling, bruising, and occasionally temporarily low eyelid. It is not yet known whether face lift has a detrimental effect during pregnancy and lactation. For this reason, it is recommended not to use face lift during pregnancy and lactation. It should not be used in people with diseases of the nervous-muscular system (such as Eaton-Lambert syndrome, myasthenia gravis).
Filler
Do you know why we like fleshy lips? Because the lips are full of beauty and beauty as well as representing youngness, attractiveness, fertility, sexiness, femininity. One of the most important complementary elements in the beauty of the face is that the lips are beautiful in terms of completing the integrity of the face is very important. The size, fullness and thinness of the lips should be consistent and proportionate with the other elements of the face, nose, cheeks, eyes, eyebrows, jaws and teeth. Thick and full lips are the expression of youth as well as thin and relaxed lips are the expression of old age. The lips have a very important place in the aesthetic appearance of the face. For this reason, lips look alive and full strengthens the expression of youth and health on our face.
In some people, the lips are inherently thin and non-prominent. In others, depending on the aging of the tissue melts and wrinkles to lose fullness and vitality. The decrease in the brittleness and fullness of the tissues is the result of the reduction and melting of the amount of skin, muscle and fat on the face. The face skin, as well as other structures, cheeks, neck skin, eyelids, eyebrows and of course lips hang down with gravity. With the sagging of the lips, the lower teeth become visible when they laugh and speak in such patients, the appearance of the lower teeth (dental show) is the expression of old age. On the contrary, when young people talk and laugh, the upper teeth are more visible, and the lower teeth are invisible. This is the young facial expression.
Whom is it applied? It is preferred by those whose lips are structurally thin or thinned over time, who want to strengthen the expression of youth and beauty on their face with thick and full lips, or those who want to have lighter and more fleshy lips. Thin and wrinkled lips as well as those of the black race, such as patients with thicker lips than normal thinning operations are also performed. In addition, patients with lip cancer due to lip cancer have to be taken again to make lip surgery operations, such as lip repair surgery is important in terms of aesthetics as well as function.
How it is carried out? The aim is to thicken the lips and make their contours more prominent, reduce wrinkles and make them fleshier. However, preserving a natural appearance is one of its main objectives.
There are many methods for thickening lips and plumping wrinkles. We can examine these methods in 3 main groups.
1. Foreign fillers:
Liquid Silicone and Silicone Oil: Injection into facial tissues was popular in the 1960-70s. Liquid silicone and silicone oil, which has been used for years in lip thickening, are now banned by the US Food and Drug Administration (FDA) for use as an aesthetic filler and are not banned and used directly because of the complications it contains and cause various complications in patients. The application here is different from that performed in breast surgery. In breast surgeries, the gel is placed under the breast in a thick silicone sheath and removed if desired. It contains no drawbacks. However, when it is injected into the lip, this silicone fluid is spread between the tissues and makes it impossible to remove. It causes inflammation of lip tissue and also causes deformity in patients because it is constantly displaced. In our country, some unauthorized persons make secretions of silicone in some hotel rooms, houses and some beauty salons. For these reasons, caution should be exercised. In our country, no plastic surgeon applies liquid silicone or silicone oil injection to the face.
Temporary Fillers; Collagen is harmless materials composed of substances such as hyaluronic acid. Collagen is obtained from cowhide. With the emergence of mad calf disease, its use has almost ceased. There is also the risk of allergies because it is animal protein, not used without testing. Hyaluronic acid is also the most widely used material for the treatment of facial wrinkles. When Hyaluronic acid is used, called Restylane®, which is injected with a needle, the application is very simple, and the patient immediately returns to normal life. 4-6 months after the application will be permanent and should be repeated at the end of this period. It is chemically obtained and does not cause allergies, it is risk free. These substances are injected immediately into the skin. They are used to erect and clarify lip edges. They are insufficient in cases where more thickening is required. 4-6 is thrown away from the body by melting. So the effect is temporary and should be done every 6 months.
There are many chemicals that are made with this type of needle and are not excreted from the body, which provides permanent improvement. They can remain in the body for 4-5 years. Such injection-type applications are extremely simple. Immediately after the patient is able to resume normal daily life.
Other solid form fillings; These are materials that are placed in a small operation, not injectors, like injected ones. Alloderm is obtained from human skin. It disappears within 1 year after being placed. The material called Gortex is permanent. It is inserted into the lip through small incisions made from the lip edges. It remains as it is and retains its shape. The drawback is that it can go out by piercing the lip over time.
2. Using tissues of the patient as filler
Lip thickening and augmentation can also be done with tissues taken from the patient’s own body. These are:
Fat Cells: Fat cells taken from the patient’s own body are injected into the lips and other wrinkles if necessary after careful treatment. 20-30% of the cells put in 3-6 months remain. When the process is repeated several times, effective lip thickening can be achieved. There is no harm does not include risks such as allergies, etc .. The procedure is performed under local anesthesia. Swellings go away in a few days. Extremely easy application. It is very effective in cases where fullness is required.
Skin cells; The subcutaneous tissue and fat cells from the patient’s own body, for example from the skin that has been removed from the skin during abdominal or breast reduction surgery, or from the scar tissue that had previously been left in the body, are mixed and injected into the lips in the form of tissue cocktail. In this method, it is necessary to repeat 2-3 times because the cells put in are partially melted. It is harmless and one of the best methods. There is no risk of allergies.
3. Reforming lips with surgery. It is possible to thicken the lips with simple operations without using filler. Many techniques are available. The plastic surgeon chooses the appropriate method according to the patient’s lip structure and condition. The best and lasting results are obtained by these techniques. It is not temporary like others. Recovery is completed in 2-5 days. These are simple operations performed under local anesthesia.
Seagull wing technique: The aim is to expand the lip by moving the lip edge upwards. The incision is made from the upper lip edge and the upper part to be raised, and then the remaining skin is removed and the lip skin is sewn on the upper skin and the pink part of the lip is raised upwards. This technique is an excellent solution for patients with very thin lips and very wrinkled lip edges. The result is permanent.
Bull’s horns or lip lift under the nose: Just below the nose of the nose in the form of bull horn cut in accordance with the amount of skin is removed and the skin is re-stitched up the lip hangs. A thin incision remains under the nose. This technique is also used to shorten this distance in patients with large upper lip skin. This ensures that the upper teeth are visible. The result is permanent.
Paris lip: Lip edges and the upper lip called philtrum under the nose, the filler of the edges of the pit or fat cells made by clarifying the name given to the lip.
V-Y technique: The upper and lower lips are cut through the mouth through the V-shaped incisions and then the Y-lips are plumped. The result is permanent. Constant.
Lip Suspension: Through a small incision made through the nose, the stitches are passed through the lips and the lip is lifted upwards. There’s no visible sign. It is a highly effective method.
The aesthetics of the lips, which are one of the most important elements in terms of completing percentage integrity, are very popular due to their effective results and simple application methods.
